NBI Clearance (Philippines)

Note: The purpose of this clearance is for entry corrections in the birth certificate. Also, application of an NBI clearance for the sole purpose of "Change of Name" is not entertained at the NBI satellite offices. You must apply at the main office.

What to Bring: ID, Cedula, black pen
Location: NBI office, 5th floor, Victory Mall, Monumento, Caloocan
Completion Time: 3 working days
Total Cost: (Php 165.00)

i. Present your ID at the entrance and get an application form. Fill it up and don't forget to indicate the purpose. Follow the guides posted on the encoding/writing area.
ii. Line up "Step 1: Data Encoding." They will verify the details that you wrote on the application and give you the payment slip depending on the purpose you placed on the form.
iii. Line up at "Step 2: Cashier." The movement of the line is slow but there is significant progress when it moves.
iv. Once you're inside, you are not going to line up at the long queues. The payment for the purpose of "Change of Name" is located at a different cashier. Ask the people inside as to what window number you should pay at. In that way, you won't waste time lining up.
v. Go to "Step 3: Finger Printing." There are several stations there, choose any. The person in charge will be responsible in placing your prints in the form. Pay him 2.00 for the wet tissue that is supposedly for cleaning your fingers.
vi. Line up at "Step 4: Imaging Section." Smile and don't say cheese. I'm not sure if they discourage people from smiling.
vii. Go to "Step 5: Data Encoding." Make sure that the details you wrote in the application is understood by the person behind the window. They are very strict with the details you place if you are applying for the purpose of "Change of Name." You need to place the name you are changing and the new name. There should be precision in each character. Even ("") double quotes are not forgiven.
viii. You will be given a stub which also contains the date when you could retrieve the clearance.

Local Birth Certificate (Philippines)

Note: The purpose I applied for was for the Change of Name

What to Bring: ID, Black Pen, (If your NSO birth certificate is different from that of the local, bring a photocopy of both)
Location: Local Civil Registrar (Manila City hall for me)
Completion Time: 2 working days
Total Cost: (Php 30.00)

i. Line up at "Step 1". Ask for a form and fill it up. Indicate the purpose as to why you are applying.
ii. Have it checked at the counter in "Step 1." If there are no more problems, they will ask you to wait until your name is called at "Step 2."
iii. Present a valid ID once you are called. Explain to the person the purpose as to why you are applying for a birth certificate if they ask. They will give you a stub which you would need to pay at "Step 3."
iv. Pay at "Step 3." The person in charge will collect the money and write the estimated date and time you could retrieve the local copy of your birth certificate. It usually takes another day before you could get it.
v. Present your receipt at "Step 4." Wait for your name to be called. Then that's it.

Affidavit of unemployment (Philippines)

Note: The purpose of this affidavit is for entry corrections in the birth certificate

What to Bring: ID, Information on what needs to be written
Location: Law firm (with a computer so that they already have a prepared template)
Completion Time: Less than an hour (or even < 30 mins.) if they already have a template and the person who will notarize is there
Total Cost: (Php 150.00)

Go to a law firm and ask for your needed affidavit. It is needed that you indicate in the document the purpose (Change of Name) and that you have no pending administrative/criminal case.

They will make it for you (3 copies) and notarize it then and there.

Community Tax Certificate - Cedula (Philippines)

Getting a cedula is easy.

What to Bring: ID, Black Pen
Location: City hall (QC hall for me)
Completion Time: Less than an hour
Total Cost: (Php 59.40)

All you need to do is fill up a certain form and line up.
i. Get a cedula form and fill it up
ii. Line up and wait for your turn. Prepare your id and the amount needed.
iii. Once your called, that's it. Just give them the information that they need
