Local Birth Certificate (Philippines)

Note: The purpose I applied for was for the Change of Name

What to Bring: ID, Black Pen, (If your NSO birth certificate is different from that of the local, bring a photocopy of both)
Location: Local Civil Registrar (Manila City hall for me)
Completion Time: 2 working days
Total Cost: (Php 30.00)

i. Line up at "Step 1". Ask for a form and fill it up. Indicate the purpose as to why you are applying.
ii. Have it checked at the counter in "Step 1." If there are no more problems, they will ask you to wait until your name is called at "Step 2."
iii. Present a valid ID once you are called. Explain to the person the purpose as to why you are applying for a birth certificate if they ask. They will give you a stub which you would need to pay at "Step 3."
iv. Pay at "Step 3." The person in charge will collect the money and write the estimated date and time you could retrieve the local copy of your birth certificate. It usually takes another day before you could get it.
v. Present your receipt at "Step 4." Wait for your name to be called. Then that's it.
