PNP clearance (Philippines)

Note: The purpose of this clearance is for entry corrections in the birth certificate (Change of Name)

What to Bring: ID, Black Pen, Cedula
Location: PNP office? (I went to UN HQ, Manila)
Completion Time: Less than an hour
Total Cost: (Php 160.00 - Php 210.00)

i. Get and fill up the form.
ii. Pay at the cashier. Remember to indicate the purpose as to the reason why you are availing it.
(Employment, Change of Name, etc..) 100.00 Php
iii. Submit the accomplished form and receipt to a person inside. Pay 50.php and sit and wait.
iv. When your name is called, make sure to verify the details in the form. Mine had a problem with the indicated purpose. It should have been "Change of Name" instead of "Employment"
v. Line up for a photo session, which costs, 10.00 PhP. Then that's it.
